Also, am I the only one who saw that Sue in the top panel and think, "dear gracious, what on earth is that monstrosity?!?"

What you are missing is that the doctor as a time lord is pretty damn hard to kill. Basically every time a time lord is sustaining a fatal wound he regenerates.
This regeneration process heal the time lord from all the damage caused to him, but also change the way he looks and even some of his or her demeanor and character. (The actor that plays the doctor changes)
This fact allows the show to switch the actor that plays the doctor every couple of years and is the reason why the show have survived for 50 years.

The doctor from part II of ensign sue is the tenth doctor and the doctor in part three is number eleven.
By the way in the show number eleven recently regenerated to number twelve.

the first actor to play the doctor was william Hartnell. He played the part of the doctor from 1963 to 1966. Near the end of his run his tenure as the doctor his health started deteriorating to the point that he had problems to keep acting. The producers found themselves in a situation where they where about to lose their main actor and therefore the show.
 then they came up with the idea: The doctor isn't human but a very powerfull alien. What if he had the power to change form? this way when Harnell leaves they can have someone else to play the doctor and keep the show going. The rest as they say is History.
